How to obtain vital record

Vital Records could be obtained in person or by mail. Maine Vital Records does not accept credit cards or online orders.

  • For obtaining certificates in person, walk-in request with payment by cash or check to vital records office located at 220 Capitol Street in Augusta, Maine.
  • For obtaining record through mail enclose a copy of photo ID and a long, self-addressed, STAMPED envelope with the request.
